Origin
In the early 1800s many Americans used handmade tools with ax-heads to chop down trees and build houses. These handmade tools were not always of good quality, so it could happen that, during energetic chopping, the loose ax-head would dangerously fly off the handle. Hence the use of the expression to describe a risky "loss of control".
